Job Summary: Memorial Healthcare System seeks a skilled Case Manager to assist with patient care transitions, discharge planning, and interdisciplinary rounding at Memorial Regional Hospital in Hollywood, Florida.

This role requires a registered nurse with a minimum of one year of experience in hospital-based case management practice or five years in healthcare. The ideal candidate will possess strong communication and leadership skills, with the ability to navigate complex healthcare systems and coordinate patient care.

Responsibilities:

• Works with patients and families to navigate the healthcare system, including post-acute care options and insurance plans.
  
• Collaborates with the interdisciplinary team to evaluate patient needs and develop effective care plans.
  
• Organizes and facilitates care conferences with patients, families, and care team members.
  
• Participates in discharge planning, including coordinating patient transfers and community resources.
  
• Provides education and resource referrals to patients and families.
