Position Overview We are seeking reliable and dedicated Home Health staff ( Therapists OT,PT,OTA,PTA) to join our growing team. The ideal candidate will provide skilled care and support to patients across various settings and must have the flexibility to work weekdays as required by patient needs. Physical Therapist (PT) / Physical Therapist Assistant (PTA) Responsibilities: Perform evaluations and develop treatment plans to improve mobility, strength, and balance. Deliver therapy sessions focused on functional independence and fall prevention. Document progress and communicate with interdisciplinary team. Document visits within 24 to 48 hours of seeing the patient Educate patients and caregivers on home exercise programs. Must participate in weekend rotation as needed. Requirements: Florida PT or PTA license, CPR certification, valid drivers license. Home health experience preferred. Occupational Therapist (OT) / Occupational Therapy Assistant (OTA) Responsibilities: Conduct functional evaluations to maximize patient independence. Provide training in ADLs, adaptive equipment, and energy conservation. Document sessions and update care plans in EMR. Collaborate with caregivers and interdisciplinary team. Must be able to work weekdays/weekends. Requirements: Active Florida OT or OTA license. CPR certification, TB test, and drivers license Document all visits and services accurately and promptly in the EMR system. Document visits within 24 to 48 hours of seeing the patient Follow infection control and safety protocols at all times. Communicate effectively with the interdisciplinary team to ensure continuity of care. Requirements Active Florida license/certification (PT, PTA, OT, OTA). Resume Current CPR certification. Negative TB test or chest X-ray. Valid drivers license and reliable transportation. Clear background check and eligibility to work in the U.S. Ability to work flexible hours including weekdays, weekends, and some holidays. Strong communication, organizational, and problem-solving skills. Preferred Qualifications Previous experience in home health or long-term care. Familiarity with EMR systems.
