Job Summary
Patient advocate and liaison responsible for assisting patients
with navigating the continuum of care, providing emotional support,
and promoting their ability to understand and participate in the
healthcare process.

**Responsibilities:**
--- Collect and assess data to identify quality trends or actual
sources of risks to patients and employees
--- Collaborate with multidisciplinary teams and committees to
improve quality outcomes
--- Provide emotional support, counseling, clinical education, and
expert guidance to patients and families
--- Assist patients and families with resolving financial,
psychosocial, functional, and administrative issues
--- Intervene as necessary to advocate for patient and family
needs
--- Continuously review and evaluate patient progress as outlined
within the plan of care
--- Communicate plan of care to patient and family and solicit
concerns, questions, and issues for resolution
--- Serve as a liaison between patient and clinical staff,
administration, physicians, and community/external resources to
coordinate/maximize resources and identify/resolve barriers to the
plan of care
