Job SummaryWe are seeking a skilled Navigator - Patient and Family to join our team at Memorial Healthcare System. As a key member of our healthcare team, you will play a vital role in ensuring that patients and their families receive the highest level of care and support.Key Responsibilities

• Collect and assess data to identify quality trends and risks to patients and employees.
• Collaborate with multidisciplinary teams and committees to improve quality outcomes.
• Provide emotional support, counseling, and education to patients and families to promote their understanding and participation in the healthcare process.
• Assist patients and families with resolving financial, psychosocial, functional, and administrative issues.
• Advocate for patients and families, intervening as necessary to ensure their needs are met.
• Communicate patient progress and plans to patients and families, soliciting concerns and issues for resolution.
• Serve as a liaison between patients, clinical staff, administration, physicians, and community resources to coordinate and maximize resources.Requirements
  • Accredited program in nursing or social work.
  • BLS certification from the American Heart Association.
  • Registered Nurse Compact License or equivalent.
