Job SummaryWe are seeking a skilled Navigator - Patient and Family to join our team at Memorial Healthcare System. As a key member of our healthcare team, you will play a vital role in ensuring that our patients receive the highest level of care and support.Key Responsibilities:

• Provide emotional support and counseling to patients and families to promote their ability to understand and participate in the healthcare process.
• Assist patients and families with resolving financial, psychosocial, functional, and administrative issues.
• Collaborate with multidisciplinary teams to identify and address quality trends and risks to patients and employees.
• Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.Requirements:
  • Accredited Program (Required)
  • BLS American Heart E-Card (BLS AHA ECARD) - American Heart Association (AMERICAN HEART)
  • Registered Nurse Compact License (RN LICENSE COMPACT) - Compact RN Multistate
  • Registered Nurse License (RN LICENSE) - State of Florida (FL)Working Conditions:
